Breathing Exercises for High Blood Pressure: What Doctors Now Recommend

In 2021, the FDA cleared a prescription breathing device called RESPeRATE for the treatment of hypertension. The device guides users to breathe at 10 breaths per minute for 15 minutes daily. Clinical trials showed it reduced systolic blood pressure by an average of 15 mmHg — a reduction comparable to some antihypertensive medications.

Then researchers realised you don't need the device. The breathing pattern is what matters. And you can do it for free.

How Breathing Affects Blood Pressure

Blood pressure is regulated by the autonomic nervous system via baroreceptors (pressure sensors in the arteries) and the renin-angiotensin-aldosterone system. Chronic sympathetic activation — the fight-or-flight state — raises blood pressure by constricting blood vessels and increasing heart rate.

Slow, deep breathing activates the parasympathetic system via the vagus nerve, which dilates blood vessels, reduces heart rate, and directly lowers blood pressure. Regular practice also reduces the sensitivity of the baroreflex, making the cardiovascular system less reactive to daily stressors.

Additionally, nasal breathing and slow breathing increase nitric oxide — a powerful vasodilator that relaxes blood vessel walls.

The Most Effective Technique: Slow Paced Breathing

Multiple meta-analyses confirm slow paced breathing at 6–10 breaths per minute as the most evidence-backed intervention. The most-studied pattern:

  • Inhale for 5 seconds
  • Exhale for 5 seconds
  • 10 breaths per minute
  • 15 minutes per day

A 2021 study in the Journal of the American Heart Association found that 6 weeks of this practice reduced systolic BP by an average of 9 mmHg and diastolic by 4 mmHg in people with high-normal to Stage 1 hypertension. A 2023 University of Colorado study found 5–10 minutes of slow breathing daily for 6 weeks produced reductions of 10 mmHg systolic — comparable to aerobic exercise.

Extended Exhale for Acute Reduction

When BP is acutely elevated (stress, white coat hypertension in medical settings), an extended exhale pattern produces faster reduction:

  • Inhale for 4 seconds
  • Exhale for 6–8 seconds
  • 5–10 rounds

The longer exhale maximises vagal activation and can produce measurable BP reduction within 2–3 minutes.

Important Medical Context

Breathing exercises are a complementary intervention — they do not replace medication if medication has been prescribed. Never stop prescribed antihypertensive medication without consulting your doctor. Breathing exercises are most appropriate as:

  • An adjunct to lifestyle interventions for high-normal or Stage 1 hypertension
  • A stress management tool that addresses one of the primary drivers of elevated BP
  • A first-line intervention under medical guidance before medication is required

If your blood pressure is regularly above 140/90, work with your GP. Breathwork is powerful but not a substitute for professional medical care in moderate-to-severe hypertension.

Building a Daily Practice

Consistency matters more than session length. Even 5 minutes daily produces measurable changes over 6–8 weeks. The best time is whatever time you'll actually stick to — morning, lunch, evening. Lying down is fine and may produce more consistent results than seated practice.
