Diaphragm Exercises: How to Strengthen Your Primary Breathing Muscle
The diaphragm is the most important muscle in your body — and most people have a weak one from years of chest breathing. Here's how to train it and why it matters.
The diaphragm is a dome-shaped muscle at the base of the lungs. It's responsible for 70–80% of respiratory effort, and it connects to your spine, ribs, and abdominal fascia in ways that affect posture, core stability, pelvic floor function, and nervous system regulation. Most people, through years of sedentary lifestyle and stress-induced chest breathing, have a chronically weak, shortened diaphragm. The consequences ripple outward.
What Happens When Your Diaphragm Is Weak
- Chronic shallow breathing — reducing oxygen delivery and raising CO₂ sensitivity
- Increased use of accessory muscles (neck, shoulders, upper chest) — contributing to neck pain, tension headaches, and shoulder tightness
- Reduced core stability — the diaphragm forms the roof of the core canister; when it dysfunctions, the whole system compensates
- Elevated sympathetic tone — shallow breathing keeps the stress response partially active
- Poor posture — a weak diaphragm contributes to forward head posture and thoracic kyphosis
Exercise 1: Diaphragmatic Breathing Drill
This is the foundation — learning to recruit the diaphragm rather than the chest.
- Lie on your back, knees bent, feet flat. Place one hand on your chest, one on your belly.
- Breathe in through your nose for 4 counts. Only the lower hand should rise. Chest hand stays still.
- Breathe out slowly for 4–6 counts. Feel the belly fall.
- 10 repetitions. Progress to seated, then standing.
Practise this 10 minutes daily for 2 weeks. You're retraining a neuromuscular pattern, not just stretching — it takes repetition.
Exercise 2: Segmented Breathing
This trains the full respiratory range — belly first, then ribs, then chest — in one coordinated breath.
- Sit upright. Inhale slowly and fill the belly first (lower hand rises), then let the ribs expand (feel them widen sideways), then allow the chest to rise slightly.
- Exhale in reverse: chest falls, ribs draw in, belly falls last.
- 8 rounds. The goal is fluid coordination between all three zones.
Exercise 3: Resistance Breathing (Pursed-Lip or Inspiratory Load)
Adding resistance to inhalation specifically strengthens the diaphragm and inspiratory muscles.
- Pursed-lip resistance: Exhale through slightly pursed lips (as if blowing out a candle from 30cm away). The resistance on exhale increases diaphragm work on the following inhale.
- Straw breathing: Inhale through a standard drinking straw for 3–5 minutes. The resistance directly loads the diaphragm. This technique is used in respiratory physiotherapy and has clinical trial support for improving respiratory muscle strength.
Exercise 4: 90/90 Diaphragm Reset
Used by movement specialists to restore diaphragm position and function.
- Lie on your back, hips and knees at 90 degrees with feet on a wall or chair.
- Exhale completely — "shhhh" sound — until fully empty. Feel your lower back flatten into the floor.
- Inhale through your nose, expanding belly and lower ribs. Hold 2 seconds.
- Exhale completely again. 5 repetitions.
This resets the diaphragm to its optimal domed position, which is compromised in people who spend long hours seated.
Exercise 5: Box Breathing with Diaphragm Focus
Standard box breathing (4-4-4-4), but focus exclusively on belly movement — ensuring the diaphragm is doing the work throughout. This combines the well-evidenced nervous system benefits of box breathing with diaphragmatic training.
How Long Until You Notice a Difference?
Subjective improvement in breathing ease, stress resilience, and reduced shoulder tension typically appears within 2–3 weeks of daily practice (10 minutes). Respiratory muscle strength measurably increases within 6–8 weeks. Postural improvements take longer — 3–6 months of consistent practice.
